Standards of Care Teaching Sheet

Here is what we use at The Portland Clinic. It is filled out before patients visit and reviewed with patients to explain what diabetes standard of care targets are, if they need any updated and if they are meeting the targets. We find it to be a great teaching tool that allows patients to ask questions about options, whether they are lifestyle changes and/or medication changes that would help they achieve the targets and then at bottom of page they can set a goal.
